How to Order Gift of Choice Award Packages

The following instructions and forms are provided for your convenience when ordering Gift-of-Choice award and gifts for Employee Recognition. 

When ordering Service Awards or Holiday Gifts, please go to these Special Instructions:

Please call if you have special requirements for your employee recognition award packets, such as custom letters to be included with your packets, or if you need any assistance placing your order. We're always available to help make ordering employee recognition award packets easy!

The Order Form  is an easy-to-use interactive PDF form you can download and save on your computer. When completed, you simply submit your order to Select-Your-Gift for processing.

Order-form-thumbOrder Form: Please SAVE this PDF document on your PC. Then re-open it to complete it. Use this Interactive PDF Form to order your Award packets. Once you have completed your order, you can print it, and then email it by just clicking a button on the form email-this-order-img . Be sure to save a copy of this form on your computer for future re-orders.

Credit-Card-form-thumb-1Paying by Credit Card: Use this form when paying for your order by credit card. This is only needed to initially provide your credit card information, or when something regarding your credit-card has changed. It is not normally needed for re-orders. An Invoice will be mailed along with a credit card transaction receipt.

Credit-Appl-Form-ThumbCredit Application: Please use this form if you want us to bill by Invoice for your employee recognition award packets. This form is only required with your first order (and only if requesting credit). All information is held in the strictest confidence. Note: Alternately, you can provide your own credit information form, if available.

Using the above ORDER Form, follow these
easy steps to complete and place your order:

  • Gift Packet Quantities - Indicate the quantity needed for each Gift Catalog Package Level
  • Choose your themed stationery and matching seals. Note: Stationery is normally provided blank, ready for printing recognition certificates. One blank, themed sheet is included with each packet. Click to see images/samples of each stationery sheet and seals
    • Optional custom printing of certificate: Optionally, we can print your Recognition or Gift message on the provided stationery. Please call for details on custom printing, including pricing and minimums.
    • Optional Personalization: If you want us to personalize your certificate-sheets with each employee's name, please email your list of names in an excel file to ensure accuracy. Please contact us for excel file format, pricing and minimums.

Completing your order:

Date Needed: In the Special-Instructions section of the Order form, please tell us if you have any special shipping requirements and the date needed for your employee packets.

Sending your Order: Please Email or Print/FAX the Order form. Include your preferred payment information: Credit Card form, or Credit-Application-form with first order. Note: When using the Interactive PDF Order-form, simply click the top-right button to submit the form via Email.

What to expect after placing your order:

We will call or email to confirm receipt of your order.

When your order ships, you will receive an email along with a UPS tracking number
